Comprehensive approach to early detection of echinococcosis and alveococcosis in endemic foci: Analysis of behavioural risk factors and screening ultrasound examination

ABSTRACT
Introduction: Echinococcosis and alveococcosis remain significant public health problems in endemic regions, particularly in rural areas where close human–animal interactions and limited preventive practices facilitate transmission. Objective: The study was devoted to the analysis of epidemiological features of cystic echinococcosis and alveolar echinococcosis in the Kara-Kuldja district of Osh region of the Kyrgyz Republic and the development of recommendations for improving diagnosis and prevention of these parasitic diseases. Material and Methods: From July to August 2023, epidemiological observation was conducted, covering 1277 residents from 20 settlements of the Kara-Kuldja district. The methodology included screening ultrasound examination of the abdominal organs and a questionnaire survey to identify behavioural risk factors. Results: The results showed a low prevalence of alveolar echinococcosis – 0.31% (four cases; 95% CI: 0.09-0.80%) and cystic echinococcosis – 0.98% (12 cases; 95% CI: 0.52-1.44%). The practice of home slaughter of livestock turned out to be a significant risk factor for alveolar echinococcosis (Odds Ratio = 3.85; 95% CI: 1.12-13.27; p=0.03), and the absence of regular dog deworming demonstrated a tendency to increase risk (OR=2.94; p=0.08). Among participants, a high frequency of unsafe practices was identified: 62.3% of dog owners allowed the dogs to roam freely, 55.2% reported animals having access to gardens, and 37.2% of respondents lacked basic knowledge of these zoonoses. Ultrasound confirmed its value, but its limited sensitivity (60-90%) indicated the necessity of complementing it with serological tests and imaging methods such as computed tomography or magnetic resonance imaging. Conclusins: The study emphasised the importance of a comprehensive approach including regular screening, veterinary control and educational programmes. Recommendations envisaged obligatory deworming of dogs at least four times per year, safe disposal of slaughter waste and raising public awareness. These measures could significantly reduce morbidity in endemic regions, contributing to improved public health.
